THE EDEN TOWNSHIP HEALTHCARE DISTRICT BOARD OF DIRECTORS HAS SUSPENDED GRANT APPLICATION SUBMISSION AND GRANT FUNDING UNTIL FURTHER NOTICE.
 
Since 1998, the Eden Township Healthcare District has provided grants to non-profit and government agencies to improve the health and well-being of local residents. This is the first time that significant funds have been available specifically for central Alameda County residents, enabling local agencies to boost services to people of all ages and needs.
 
These grants have proven to be a valuable resource to agencies and to the public.
 

Potential Grant Applicants Please Note:

The Eden Township Healthcare District Board of Directors will be reviewing funding priorities and guidelines for the Community Health Fund and making revisions in 2010. While current priorities and guidelines will remain in place for the Fall 2009 cycle of grants, the District is interested in receiving and considering proposals for programs and projects that take new and different directions on behalf of the health of residents of the Eden Area.

 

Several key elements of a new direction can include:

·      Focus on the health needs and solutions for an entire area such as a neighborhood or community.

·      Recognition that good health is influenced by the physical, social and economic environment in a   neighborhood or community.

 

Capital projects will be considered for funding as long as they match funding priorities and meet other eligibility requirements.

FUNDING PRIORITIES
The following priorities are based on community needs and are set forth to help guide interested organizations in determining eligibility for funding through the Community Health Fund project grants:

  • Efforts to make health care more accessible and affordable, especially to under-served residents of the Eden Township Healthcare District;
  • Delivery of health-related services to high-risk/special needs populations, with specific and measurable outcomes that advance the health and wellness of these populations;
  • Programs that address health concerns of the Ashland and Cherryland communities;
  • Programs that demonstrate collaboration with other organizations to meet the identified needs above.

GRANT LEVELS
Eligible organizations may apply for one of two grants in any fiscal year (July 1 - June 30):

  • Small Grants, up to $5,000
  • Large Grants, up to $75,000

SPECIAL PROJECTS
The Eden Township Healthcare District looks for opportunities to help create strategic alliances among individuals and organizations, harnessing their collective power to confront, explore and solve significant health care issues in the Eden Township Healthcare District.

 
Since launching the Community Health Fund in 1998, the Eden Township Healthcare District Board of Directors has seen a great need to coordinate services to the community, as many organizations have similar programs that could have a more profound impact by working together. At the same time, the Board has expressed its desire to provide direct services that focus on a defined health concern in the community over a longer period of time. To that end, the Board has now expanded its program to consider special, long-term projects known as Focused Projects. Focused Projects will be considered outside of the regular Community Health Fund grant review process.
 
Based on their latest Community Needs Assessment, the District has identified two areas of special needs within the District. These areas are under consideration for research, development and multi-year investment at the direction of the Board of Directors:
  • PROJECT 1: To improve access to primary care services in the Ashland Community.
  • PROJECT 2: To reduce the rate of alcohol-related injuries and deaths in Alameda County.
